Here's what to complete, bring, and expect before your visit.

To help make your check-in process as smooth as possible, please complete your new patient paperwork before your appointment.
You will receive a text message with a link to your patient forms approximately five days before your scheduled visit. You may also access the forms below.

2. What to bring
Photo ID and Insurance Card
Your current cards make check-in easier.
Copayment and referral, if needed
Check your plan for any referral requirements.
Relevant Medical Records
Recent CT scans, test results, or hospital records.
Medication List
Or bring your medication bottles with you.
Insurance & Billing
Before your visit, contact your insurance plan to confirm that your physician is in network and whether you need a referral or prior authorization. Please be prepared to pay any applicable copay or deductible at your appointment. Call our office if you have questions about billing or payment arrangements.
