I Have an Appointment, Now What?

  1. Print and fill out the Patient Registration Forms. Please remember to give us complete and up to date addresses and telephone numbers. 

  2. Print and fill out Medical History Form. This form helps us understand the nature of your current condition so we are able to treat you more effectively.

  3. Prepare for your Office visit. In addition to completing the above forms, we will also require the following information for your first visit:

    • Driver’s license or other valid Government –issued Photo ID

    • Insurance care

    • Co-payments and outstanding balances

    • If applicable, any referral forms

    • Any X-Rays, MRI films, CT Scans,

    • Any information on recent hospitalizations

    • Any pertinent medical history such as lab results.

    • Complete list of medications you are currently taking. Otherwise, bring all medications with you.

  4. Verify our participation with your insurance plan. Please check with either our office or your insurance plan to make sure we accept your insurance plan. Have your insurance card handy so you can give accurate information regarding your current plan.

  5. Keep in mind that many of our patients are allergic to perfumes and strong scents, so please be sure to wear or bring no overwhelming scents into the office.

  6. Get Directions to Our Office. Visit our locations section to see a map of our office and get personalized driving directions.

  7. We look forward to treating your sleep, shortness of breath, cough, snoring, sleep apnea, and/or other related issues! Please arrive 20 minutes early if you have not filled out the forms. Otherwise, you may arrive 5-10 minutes early. Please call if you are running late for your appointment. If we do not hear from you within 30 minutes of your appointment time, your appointment may be cancelled and given to another patient.
